Abstract

Background: Every year, an estimated 15 million babies are born preterm and this number is rising. Pre-term birth complications account for 35% of the estimated 3.1 million global neonatal deaths and are the second leading cause of death in children under 5 years of age. Objective: The objective this study was to assess factors associated with preterm birth and its immediate outcome from 2011 to 2015 in Addis Ababa public hospitals having neonatal intensive care unit, Addis Ababa, Ethiopia, 2016. Methodology: Retrospective cross sectional study was conducted from March 1 to April 1, 2016 on 384 preterm births. After random selection of the three hospitals from the six Addis Ababa Public hospitals having Neonatal intensive care unit, systematic sampling was employed to select study units from admission log book of the neonates. Data were collected using structured questionnaire by trained three Bsc nurses. Finally data were entered to EPIDATA3.1 and transported to SPSS 22 for analysis. Sentence, table of frequency and graphs were used to present descriptive data, and Bivariate analysis was processed and multivariate logistic regression analysis was done for the variables having significant association of p<0.2. Result: The mean gestational age of preterm birth was 32.45 (±2.903 SD). 66.1% of preterm births were spontaneous and 33.9% were indicated preterm birth. From those admitted preterm births to Neonatal intensive care unit, 45.3% were died and 54.7% discharged alive. Maternal Human immune deficiency virus (HIV) infection [p=0.003, AOR=6.446, 95%CI: (3.085, 26.279)] was one of the factors associated with both preterm birth and immediate death outcome of preterm birth. Conclusion: Maternal HIV infection and hypertension during pregnancy were associated with preterm birth; and maternal HIV infection, premature rupture of membrane, low APGAR score and respiratory distress were associated with immediate death outcome of preterm birth. Thus, concerned bodies are recommended to act over these factors accordingly so as to reduce their respective problems.
