Abstract

Background An antibody-based rapid test for tuberculosis (TB) has long been required due to technical simplicity, rapidity, low cost in comparison with culture method. The low sensitivity of microscopy methods in detection of TB bacillus among human immune deficiency virus (HIV) infected patients and children, who have difficulty to produce sputum, also necessitate for alternative methods. Objective: To evaluate the performance of Combo Serological Rapid TB test for the diagnosis of TB Methods: Cross-sectional study was conducted from January, 24 to April 2, 2014 on 210 TB suspected patients at Federal Police Commission Referral Hospital. Three sputum specimens (spot-morning-spot) and blood sample (for rapid combo TB test) were collected from participants using convenient sampling method. Sputum specimens were smeared on microscope slides and stained with Ziehl - Neelsen stain to be examined under the microscope (conventional method) for the presence of acid fast bacilli (AFB) and confirmed by the culture methods Lowenstein-Jensen (LJ) and Mycobacterium Growth indicter Tube (MGIT). The culture was considered as gold standard. Blood samples were analyzed using rapid combo TB test. Data obtained from comparative experiments were entered and analyzed in Excel and SPSS version 15. Sensitivity, specificity, negative and positive predictive values and kappa value were determined. Results:- A total of 210 TB suspected patients were involved. Of these, 106(50.5%) were male and mean age was 32.8 (SD=5.5) years. Overall, 57(27.1%) were positive by AFB microscopy, 28(13.3%) by rapid combo TB test. Among sub sample tested (n=98) by TB culture 61.2 % (60/98) were found positive. A total of 98 (46.7%) participants were tested by all three methods. Of these 28(28.6%) samples were positive by all the three methods and 60 (61.2%) samples were positive by any one of the methods. The sensitivity and specificity of Combo serological Rapid TB test in comparison with the gold standard (Culture methods) were 43.8 % and 94.6%, respectively. Furthermore, percent agreement between combo rapid test method with AFB smear and culture was 89.0% and 61.2%, respectively.Conclusion:- Combo rapid TB serological test showed lowered sensitivity and higher specificity compared with the manufacturer claim. Since this will lead to misdiagnosis, the search alternative has to continue to maximize diagnosis of TB.
