Abstract

Background: HIV infection in gynecological surgical diseases (GSDs) is associated with higher complications but epidemiological data in Ethiopia is scarce. Objective: To determine the proportion, pattern and factors associated with HIV among GSDs patients and provide useful information to other set-up. Materials and Method: This two-year retrospective cross sectional study enrolled consecutive card numbers operated from sept‘1 /2004 E.C. through 5/13/2005 E.C for gynecological surgeries in GMH. The patients‘ socio-demographic characteristics and associated factors were obtained using structured questionnaire. All Sero-positive women were linked to ART care. The outcome measures were proportion of women with HIV positive results, associated factors and mortality rate over the peri-operative follow up period. Results: The overall proportion of HIV positive result was 6.4% (39/608) with women aged 35- 45 years having the highest proportion (11.9%). Of the HIV positive patients, 18% (7/39) were on antiretroviral drugs. STIs (p<0.000), induced abortions (p<0.000), post -op pneumonia ( p< 0.000) were HIV-associated factors. Previous blood transfusion, surgeries ,benign tumors, or non-cervical gynaecological malignancies were not. There were no mortalities over the perioperative period. Almost all partner‘s HIV status were unknown. Conclusion: The proportion of HIV infection among women admitted for GSDs was high but prior knowledge of their partner HIV status was low. The study has provided a window of opportunity to see the best practice at GMH for the 100% screening. In other similar setting‘s Integration of HIV counseling and testing into gynecological services would be desirable.
