Abstract

Back ground: In individuals infected with human immunodeficiency virus, hematological abnormalities are associated with increased risk of disease progression and death. The use of antiretroviral drugs could positively or negatively affect these disorders. However, little is known about its impact on hematological and immunological parameters in patients who are taking antiretroviral drugs in Ethiopia. Objective: The study was undertaken to assess hematological profiles among HIV positive adults taking highly active antiretroviral therapy (HAART) at Black Lion Specialized Hospital, Addis Ababa, Ethiopia, 2016. Methods: A cross-sectional study was conducted from August to October, 2016 on 255 HIV infected adults who have been taking HAART for more than six months in the study hospital. Subjects whose age were greater than or equal to 20 years were enrolled in the study and had a mean ± SD age of 40.6 ± 9.4 years. Data was collected by using structured questionnaire that included variables related to socio-demographic characteristics and clinical conditions of the study individuals. Measurements of complete blood cell count and CD4 + T cell counts were made using Sysmex XT 2000i hematology analyzer and BD FACScount CD4 analyzer, respectively. P<0.05 was considered as significant. Results: There was significant increase in the mean values of CD4 count (162.5 vs 347cells/l, p<0.001), WBC count (5.52 vs 6.19×103/l, p<0.001), neutrophil (2.99 vs 3.25×103/l, p<0.001), lymphocyte (1.85 vs 3.44×103/l, p<0.05), hemoglobin (12.8 vs 14.34g/dl, p<0.001), all RBC indices (p<0.001) and platelets (223 vs 273×103/l, p<0.001) while there was a decrease in RDW (14.91 vs 13.66%, p<0.001) and RBC (4.41 vs 4.28×106/l, p< 0.001) after six months of HAART compared with the baseline. There was also significant decrease in the prevalence of anemia (41.9 vs 11.4%, p-<0.001), leucopenia (26.7 vs 16.5% p<0.001), neutropenia (16.5 vs 9.4%, p<0.05), lymphopenia (20 vs 3.1%, p<0.001) and thrombocytopenia (25.9 vs 7.1, p<0.001) after six months of HAART compared to the baseline. Conclusion: It is evident from this study that the participants have shown significant improvements in hematological profiles after taking antiretroviral treatment. Further investigation is however necessary to assess hematological profiles in HIV patients.
